Terms of Service

These terms are an agreement between you and ClaimChief, Inc. (“ClaimChief,” “we,” “us”) about using claimchief.ai and our claim filing service. By creating an account or sending us a claim, you agree to them.

1. What ClaimChief does

ClaimChief helps you get reimbursed by your health or pet insurer. You send us bills, receipts, explanation of benefits statements, and similar documents. We read them (with the help of AI and our staff), prepare a claim on your insurer’s form, sign it on your behalf as you authorize below, and submit it to your insurer by mail, email, or another channel the insurer accepts.

We are not an insurer, a health care provider, a law firm, or a public adjuster. We don’t give medical, legal, or insurance advice, and we don’t decide whether your claim is covered. Your insurer makes that decision under your policy.

Estimates of what you might get back are estimates. They’re based on the documents and plan details you give us and typical plan rules, and your insurer may pay more, less, or nothing.

2. Who can use ClaimChief

  • You must be at least 18 and live in the United States.
  • You must be the policyholder, or otherwise legally allowed to file claims for the person (or pet) who received care. For a spouse or dependent, you confirm you’re authorized to act for them.
  • You must give us accurate, current information and keep it up to date.

3. Your authorization to file and sign for you

When you sign in your profile and press send on a claim, you:

  • appoint ClaimChief as your agent for the limited purpose of preparing and submitting that claim, and any follow-up the insurer requests about it;
  • authorize us to place your typed signature on the claim form and any related authorization the insurer requires;
  • confirm that the care described was actually received, that the amounts are what was charged or paid, and that the information is true to the best of your knowledge.

Submitting a false or misleading claim is insurance fraud. We may refuse, hold, or withdraw any claim we believe is inaccurate, and we may close accounts used for fraud.

You can withdraw this authorization at any time by contacting us. Withdrawal doesn’t affect claims we’ve already submitted.

4. Your account

You sign in with a one-time code or link sent to your email. Keep access to that inbox secure: anyone who can read it can sign in as you. Tell us right away at support@claimchief.ai if you think someone else has used your account.

We may send you emails about your account and claims. These are part of the service, so you can’t opt out of them while your account is open.

5. Fees and payment

Creating an account and seeing your claim preview are free. You pay a flat fee for each claim, and only when you press send. The fee is shown on the button before you pay, and that is the amount you’ll be charged. Payments are processed by Stripe.

Prices may differ between customers and over time. We test different prices to find what works, so the fee shown to you may not match what another customer sees. You will always see your exact fee before you pay, and the price you were shown is the one you’re charged.

Your insurer pays any reimbursement directly to you. ClaimChief never receives, holds, or takes a cut of your reimbursement.

Refunds are covered by our Refund Policy.

6. Your documents and information

You keep ownership of everything you send us. You give ClaimChief permission to store, copy, process, and transmit it, including sending it to your insurer and to the service providers who help us run ClaimChief, only to provide and improve the service as described in our Privacy Policy.

You’re responsible for having the right to share what you send, including other people’s information on bills for family members. Please keep your own copies of your documents; ClaimChief isn’t a records archive.

7. Acceptable use

You agree not to:

  • submit claims for care that wasn’t received, or alter bills, receipts, or other documents;
  • use ClaimChief for anyone you aren’t authorized to act for;
  • try to access other people’s accounts or data, or probe, scan, or overload our systems;
  • upload malware or content that’s unlawful or unrelated to an insurance claim;
  • copy, resell, or build a competing service from ClaimChief.

8. Our responsibilities and their limits

We’ll prepare and submit your claims with reasonable care. If a claim is rejected because of a mistake we made, tell us and we’ll correct and resubmit it at no charge.

Beyond that, ClaimChief is provided “as is.” To the fullest extent the law allows, we don’t promise that any claim will be paid, paid in full, or processed by a particular date, and we disclaim implied warranties of merchantability, fitness for a particular purpose, and non-infringement. Insurers, the postal service, and other third parties are outside our control.

To the fullest extent the law allows, ClaimChief is not liable for indirect, incidental, special, consequential, or punitive damages, or for lost reimbursement, and our total liability for any claim relating to the service is limited to the greater of the fees you paid us in the 12 months before the claim arose or $100. Some states don’t allow these limits, so they may not fully apply to you.

9. Ending your account

You can stop using ClaimChief at any time and ask us to close your account and delete your information by emailing support@claimchief.ai. We may suspend or close accounts that break these terms or put other customers or the service at risk. Sections 5, 6, 8, 10, and 11 continue to apply after your account ends.

10. Disputes

If something goes wrong, email us first; most problems can be fixed quickly. These terms are governed by the laws of the State of Delaware, without regard to its conflict-of-law rules, and any dispute that we can’t resolve informally will be brought in the state or federal courts located in Delaware, unless the law where you live gives you the right to bring it there instead.

11. Changes and contact

We may update these terms. If a change is material, we’ll tell you by email or in the app before it takes effect. Continuing to use ClaimChief after that means you accept the new terms.

These terms, the Privacy Policy, and the Refund Policy are the whole agreement between you and ClaimChief, Inc. Questions: support@claimchief.ai.
